While filling out forms sometimes gives you something to do in the waiting room besides read our extensive collection of outdated magazines, many patients ask if there is a way to prepare ahead of time.
- Visit the Forms and Instructions Page and look for a form called "Your information" or download it from the bottom of this page.
- If you are an established patient but some of your information has changed (medications, for example) please update your information.
- Bring your insurance card and Identification
- Bring the address and/or phone number of your favorite pharmacy.
- We may be able to send a prescription through our Electronic Health Records software securely to your pharmacy so that your prescription is already prepared for pickup by the time you arrive.
- Or, we can call it in for you as you leave.
- "Walgreen's" is unfortunately not enough information for us to prepare (there's one on every corner almost!) so you may otherwise need to leave with a paper prescription and need to wait as it is filled.
